The Inclusive Health and Wellness Hub is designed to improve access to healthcare breaking the cycles of health inequity.
The Hub provides early, targeted and preventative treatments to Brisbane's vulnerable people to improve patient outcomes and reduce reliance on emergency care.
Our team of nurses, General practitioners, and allied health, dental, outreach clinics, and support and advocacy workers provide accessible, compassionate, and responsive health and wellbeing support.

Telehealth Bulk Billing Consent
Under the New Medicare Bulk Billing Scheme, it is a requirement to have your written consent to bulk bill your telehealth consultations. Please fill in the below form and submit if you agree to the assignment of the Medicare benefit directly to the health professional (bulk bill).
